She asserts and believes that there is "nothing the matter with her" and only by the most painstaking scientific search, in which asepsis, perfect clinical and laboratory technic, constant experience with microscope and complete knowledge of the pathology of the disease plays each its important part, can the proof or actual evidence of the nature of the lesion be determined. Women who present themselves to a practitioner for a certificate of freedom from gonococcic infection commonly are subjected to examination by inspection, palpation, and urinary examination. These means are obviously inadequate for the discovery of nearly all cases except the acute and florid. Most subsiding and chronic cases are not revealed by such superficial means which, so far as they go, give normal signs only, and the proof of the disease is neglected. The failure of medical examination of prostitutes to reveal gonococcic infectiousness is due to the very insidious hiding of the bacterium in folds of the mucosa or beneath the epithelium in ducts and glands. Gonococci beneath the epithelium actively promoting a small focus of infection are not discoverable by any means. It is only when they come upon the surface or can be brought to the surface that they can be obtained for examination. This is one of the reasons why a single examination is never adequate to give the basis for the opinion that a urethral mucous membrane is not infectious. Several examinations under the most favorable circumstances for finding the bacteria and the most careful observation over a long period of time with complete cooperation on the part of the patient are always necessary before the opinion of non-infectiousness can be given honestly and fairly in any suspicious...
